Friday, July 28, 2006

Cloverdale Community Schools will open for school on Aug. 15 for students in Kindergarten through grade 12. All Kindergarten students must be 5 years old on or before July 1 to be eligible to enroll and must have a birth certificate and completed immuniztion record on file the first day of school.

Kindergarten and grade 1 students must have five DTAP's, four Polio, two MMR's, and three Hepatitis B, and 1 chicken pox vaccination, if there is no history of the child having had the disease. All children entering into sixth grade should have received their second MMR before entering Kindergarten. Additional doses are not needed. However, CDC recommends that all adolescents between the ages of 11 and 12 years old receive a tetanus booster.

Additionally, Indiana state law now requires that all students grades 9-12 be immunized against Hepatitis B. Students entering grades 9 and 12 must have completed or be in the process of completing the three dose series of shots. If you are unable to receive these vaccinations from your doctor, the Putnam County Health Department and the Johnson Nichols Clinic offer free immunizations to the public. Appointments can be made by calling the Health Department at 653-5210, ext. 104 or the Johnson Nichols Clinic at 653-6171.

Students will be in attendance all day on Aug. 15 and lunch will be served. New students will need a copy of their birth certificate and immunization records. Proof of legal guardianship may be required.
