* Dec. 15, 7:24 p.m.: Grace A. Buis, 32, Greencastle, was westbound on U.S. 40 in a 2007 Saab 93T when she hit a deer.

Damage $1,001-$2,500.

Putnam County Jail

Luiz Ruiz Armando, 20, Greencastle: Residential entry, trespassing, public intoxication, minor consumption, Dec. 17, 4:30 a.m.

Keenan Richard Monnett, 28, Cloverdale: Operating while intoxicated, public intoxication, Dec. 17, 4:37 a.m.

Kaylee Jill Mullinex, 27, Greencastle: Operating while intoxicated with prior, Dec. 16, 1:26 a.m.

Greencastle Fire Department

Dec. 15, 10:37 p.m.: Alarm malfunction at 409 S. Jackson St. Cleared 10:44 p.m.

Dec. 16, 11:19 a.m.: Carbon monoxide investigation at Putnam County Jail, 13 Keightly Rd. Nothing found. Cleared 11:35 a.m.

Dec. 17, 4:44 a.m.: Assist police with cleanup after accident at intersection of Veteran's Memorial Highway and Bloomington St. Cleared 4:56 a.m.

Greencastle Police Department

* Dec. 11, 6:55 p.m.: Jessie R. Hoke, 21, Brazil, was northbound on Bloomington St. at the intersection of Martinsville St. in a 2003 Chevrolet pick-up when he was struck head on by Phil Grooms, 47, Greencastle, driving a 1998 Dodge Ram. Cody Sheldon, 18, Greencastle, was driving a 2008 Ford Explorer northbound on Bloomington St. when a tire came out of the bed of one of the pick-ups and struck his bumper.

Grooms was unconscious and required extrication from the vehicle. An evaluation at Putnam County Hospital revealed no significant injuries. He was later arrested and taken to Putnam County Jail on charges of driving while intoxicated with injury.

Hoke complained of pain to his foot and back, but signed a release at the scene. He was later taken to the Putnam County Hospital for treatment. Houston Cherry, a passenger in Hoke's vehicle, complained of pain to his knees and hand. He also signed a release, but was later evaluated at Putnam County Hospital.

The Hoke and Grooms vehicles were towed by Jerry's. Damages $10,001-$25,000.

* Dec. 12, 1:57 p.m.: Sara Ketterer, 21, St. Louis, was eastbound on Washington St. in a 2003 Jeep Liberty and stayed left to merge onto Indianapolis Rd. She began to slide due to road conditions and lost control of her vehicle. She entered the westbound lane and collided with the 2004 Mazda of Taryn E. Owens, 20, Greencastle. Owens said she attempted to move out of the way, but was unable due to road conditions.

Damages $1,001-$2,500.

Dec. 11, 2:40 a.m., N. Jackson St.: Amber Henry, 22, Indianapolis, driving while suspended with prior.

Dec. 11, 12:54 p.m., Martinsville St. and Cedar Dr.: Alisha Donaldson, 19, Roachdale, driving left of center.
