A Fillmore woman died as a result of an accident that occurred Monday afternoon in eastern Putnam County. The vehicle, which was carrying four occupants, ran off State Road 240 and traveled nearly 750 feet before catching fire due to a fuel leak.

Authorities have identified the victims of a single-vehicle accident that resulted in the death of a Fillmore woman Monday afternoon on State Road 240, just east of County Road 650 East.

Nancy C. Trivett, 66, died at the scene as a result of head trauma from the accident, the Putnam County Sheriff's Department reported.

Investigating officer Sgt. Jon Chadd said the 2012 Chevrolet Impala operated by the victim's husband, Daniel D. Trivett, 66, Fillmore, was traveling westbound on 240, and for unknown reasons crossed over the eastbound lane.

The car then left the roadway on the south side, traveling nearly 750 feet. Upon leaving the pavement, the vehicle struck a utility pole and continued westbound into a field where it ran into a parked, unoccupied farm truck.

After hitting the truck, the car proceeded down a hill before running into a creek embankment and coming to rest in the creek.

With the help of the couple's son, Terry Trivett, an occupant in the car, Sgt. Chadd was able to break the driver's seat and pull Daniel Trivett from the car moments before it exploded into flames from a fuel leak.

The report also noted that it is believed the crash occurred because of a medical issue involving the driver, however, blood tests are pending.

Both the driver and one rear seat passenger, Tiffany A. Byrd, 43, Nashville, were transported via Operation Life ambulance to Hendricks County Hospital at Danville.
