Partnership produces Bainbridge Integrated Healthcare Services

North Putnam Family Healthcare and Crossroads Care Pharmacy has announced the opening of its integrated care services to the Bainbridge community on Monday, Dec. 18 at its new location, 209 E. Pat Rady Way, Bainbridge.

North Putnam Family Healthcare hours will be Monday through Friday, 8 a.m. to 5 p.m. Crossroads Care Pharmacy hours will be Monday through Friday 8:30 a.m. to 5:30 p.m.

North Putnam Family Healthcare is a physician practice operated by Putnam County Hospital.

Previously located at 440 E. Pat Rady Way, the expanded space will now allow the practice to offer much-needed services such as lab services and radiology (coming soon), family practice, wellness physicals, DOT physicals and patient advocate services that assist patients with the enrollment in insurance and other family services offered in the community.

North Putnam Family Healthcare offers the community four providers -- Robert Heavin, MD, Cathy Clodfelter, FNP-C, Alexandria Warren, FNP-C and Jacqueline Chandler, FNP-C.

Crossroads Care Pharmacy is new to the Bainbridge area, but not new to providing wellness and pharmacy needs. Crossroads Care Pharmacy is owned and operated by Ben and Amanda Laney who also own and operate Cloverdale Drugs Pharmacy in Cloverdale. Crossroads Care Pharmacy will offer the Bainbridge area a full-service, personalized pharmacy that includes services such as medication synchronization, medication dose packaging and delivery.

Together, the partnership allows North Putnam Family Medicine and Crossroads Care Pharmacy to give the community much-needed integrated clinical services.

An open house for the community is currently being planned. More information will be released as detailed are finalized.
