PCH plans wellness screens during June 8 Farmers Market

Putnam County Hospital recently announced that staff will be on hand at the June 8 Greencastle Farmers Market to set up a free community wellness screen.

PCH will begin the screens at 8 a.m. and will take its last screen at 11:30 a.m. These will be given on a first come, first served basis.

The PCH wellness screen will include the following:

• Know Your Numbers trifold brochure: This brochure is for the individual to keep. It has all of their results/numbers from the screening. Individuals are encouraged to share this information with their primary care provider.

• Stress Questionnaire: This short questionnaire asks the individual the level of his or her stress as well as coping mechanism. The health coach at the end of the screening will look at the questionnaire and share local resources and help that is available, should the individual cope with stress in an unhealthy manner, which includes alcohol, tobacco and/or drug use.

• Worksheets for creating healthy goals and how to better prepare for one’s next doctor’s visit.

• Labs: PCH lab technicians will perform a cholesterol and glucose test using Cholestek equipment. This equipment does not require individuals to fast before the test. Items in the finger-prick test include glucose, HDL cholesterol, LDL cholesterol, triglycerides and total cholesterol. After the test is completed, individuals will receive their lab results before leaving the screening.

• Vitals: PCH clinicians will gather vital information. Information will include height, weight, BMI (body mass index), temperature, pulse and oximetry.

• Nutrition: A PCH registered dietitian will educate individuals on a healthy diet. Visuals that show how much sugar is hidden in some of our favorite drinks and what healthy portion sizes really look like will be demonstrated. Educational material will also be available.

Anyone already aware of challenges they are facing with nutrition, such as diabetes, should share this information with staff. PCH will cater to this and create focused education and material based on these needs.

• Education/Services Available: PCH staff will be on hand with literature and material to take home about the service lines, specialties and other services available at Putnam County Hospital. The goal is to make individual aware of the healthcare resources they have close to home.

• Health Coach: When the individual has completed the screening tables, a health coach will meet with him or her to review the numbers. The coach will go over, step by step, how the vitals and labs look in comparison to “healthy” numbers. The coach will talk about what the individual is doing well and also coach the individual in areas that, if improved, will help them live a healthier, more productive lifestyle.

The health coach will also review ways to set healthy goals and plan for a doctor’s visit.
