One motorist was sent to the hospital with minor injuries following a rear-end collision Tuesday morning on U.S. 40.

At about 8:15 a.m., first responders received a call that a motor vehicle accident involving multiple injuries and a possible rollover had occurred east of South Putnam High School.

Deputy Kyle Lee of the Putnam County Sheriff’s Department told the Banner Graphic a Honda CR-V driven by Lori Hoffman had pulled onto U.S. 40 from the Wildwood subdivision when it was then struck by a 2001 Ford F-150 driven by Mark Flora of Brazil.

With both heading westbound, Flora rear-ended the CR-V and lost control of the vehicle, which left the roadway and traveled up the side embankment. The truck then rested on the embankment, while Hoffman came to a stop on the side of the road.

Hoffman and two elementary-age children were unhurt, but were checked out at the scene. Lee said that Flora sustained non-life-threatening lacerations, but was nonetheless transported to Putnam County Hospital for further medical attention.

Hoffman stated that she and the children were on their way to school when the accident occurred.

The Cloverdale Police Department, Cloverdale Fire Department, Operation Life and STAR Ambulance were also called to the scene.

Jerry’s Wrecker Services was called to take the wrecked F-150 from the scene of the crash.

Though Lee said total damages had not been determined as of a call from the Banner Graphic Tuesday afternoon, the truck was certainly totaled in the crash.

He said the CR-V could be considered totaled as well, but was able to be driven from the scene back to Wildwood where Hoffman resides.
