Putnam County Jail

Shawn L. Hornaday, 27, no city listed, was booked into the Putnam County Jail at 2:50 p.m. Tuesday for failure to appear.

Jessica M. Phares, 36, no city listed, was booked at 1:10 pm. Tuesday for theft.

Vicki L. Sellers, 38, Cloverdale, was booked at 11:30 a.m. Wednesday for a warrant out of Johnson County.

Richard L. Smith, 48, Indianapolis, was booked at 12:30 a.m. Tuesday for operating a vehicle while intoxicated and public intoxication.

Putnam County Sheriff's Department

Gary Johnson of Reelsville was traveling in the southbound lane of US 231 at 5:11 p.m. Tuesday, when a pick-up truck traveling north was in the southbound lane with its turn signal on to make a turn onto CR 500N.

To avoid a head-on collision, Johnson swerved into the northbound lane. The pick-up truck struck the passenger side of Johnson's vehicle and continued down 500N without stopping.

Johnson stated the driver was an older male with grey hair and was operating an older black Ford truck. There was an estimated $1,001 to $2,500 in damages.

Floyd D. Baker was traveling on US 36E near Morton at 2:27 a.m. Tuesday, when his vehicle struck a deer causing extensive damage to the front of his vehicle. Mink's Wrecker Service towed his vehicle. There was an estimated $2,501 to $5,000 in damages.

Juan De La Torre of Chicago was traveling on Interstate 70E at 4:27 p.m. Monday, when he slowed for traffic and a vehicle operated by Jimmie L. Cantrell Jr., of Columbus, Ohio, struck the back of his vehicle. Curtis Wrecker Service towed Cantrell's vehicle from the scene. There was an estimated $5,001 to $10,000 in damages.

Jason A. Doornick, Santa Fe, N.M., was traveling east on I-70 at 5 a.m. Monday, when he lost control of his vehicle due to water on the roadway and struck a guardrail. AAA towed his vehicle. There was an estimated $10,001 t0 $25,000 in damages.

Allan J. Hamilton Jr. of Greencastle was traveling west on I-70 at 7:03 p.m. June 9, when his motorcycle struck a patch on the road causing him to lose control of the bike and slide off the road. Hamilton sustained road rash to his elbow and shoulder and a severe injury to his left ankle. He was transported to Methodist Hospital.

Curtis Wrecker Service towed his motorcycle. There was an estimated $2,501 to $5,000 in damages.

Greencastle Fire Department

Crews were dispatched to 208 E. Liberty St. at 6:34 p.m. Monday for a carbon monoxide investigation. They were back in service at 7:10 p.m.

Crews were dispatched to Columbia Street at 11:59 p.m. Monday for power lines down due to the storm. They were back in service at 1:23 a.m.

Crews were dispatched to 407 S. Jackson St. at 2:15 p.m. Tuesday for an alarm, but were disregarded before leaving the station.

Crews were dispatched to 402 W. Walnut St. at 7:07 p.m. Tuesday for a tree down due to the storm. They were back in service at 7:37 p.m.

Crews were dispatched to Percy Julian Drive near Greencastle Middle School at 7:37 p.m. Tuesday for a tree down due to the storm. They were back in service at 7:58 p.m.

Crews were dispatched to W. Rangeline Road at 8:30 p.m. Tuesday for a tree in the road. It was cleared and they were back in service at 8:53 p.m.

Crews were dispatched to 702 Main St. at 9:53 p.m. Tuesday for a tree in the road. It was cleared and they were back in service at 10:14 p.m.

Crews were dispatched to the motor vehicle accident at Zinc Mill Road and Veteran's Memorial Highway at 8:16 a.m. They were back in service at 1:29 p.m.
