PCHPCA sets November roll-out for Vital Medical Information File

The Putnam County Hospice and Palliative Care Association (PCHPCA) has announced that it will begin rolling out its Vital Medical Information File on Monday, Nov. 1.

Partnering with Putnam County first responders, the PCHPCA designed the Vital Medical Information File to help ensure that citizens receive prompt and appropriate medical care in accordance with their wishes.

The File consists of a red magnetic envelope, which would be kept on the outside of a home’s refrigerator and be readily accessible to responders when they arrive on an emergency call, or to family members or caregivers taking the person to the emergency room or doctor.

Based upon the person’s health condition, the contents of his or her Vital Medical Information File might include, but are not limited to, medications, allergies, health conditions, surgeries, pacemaker information, copies of current medical insurance cards and signed copies of advance directives.

A square, red decal with a white cross would be placed on an outside door to notify emergency responders and caregivers of the Vital Medical Information File on the refrigerator.

While all adults in Putnam County can have a Vital Medical Information File, it is conducive to the needs of senior citizens, those living alone or those living with chronic illness.

Information about how to obtain a copy of the Vital Medical Information File will be forthcoming.

The Putnam County Hospice and Palliative Care Association is a nonprofit public charity that has joined national, state and community efforts to increase the quality of life for seriously ill persons, their loved ones, their caregivers and the health care community in Putnam County.

The PCHPCA operates with the help of donations, financial support from the Putnam County Community Foundation and fiscal sponsorship by the Putnam County Hospital.
