PCHPCA: New information packets for seriously ill available

During this National Hospice and Palliative Care Month, the Putnam County Hospice and Palliative Care Association has announced the release of a new resource for seriously ill patients and their families, caregivers and health care providers.

The new resource folder offers a wealth of information including:

1. Home health care providers serving Putnam County;

2. Local home-based personal care providers;

3. Local palliative and hospice providers;

4. Local wheelchair and stretcher transportation providers;

5. General information about hospice and palliative care; and

6. Fact sheets about life-prolonging treatments.

“We are very excited to be able to offer this new patient packet,” said Executive Director Elaine Peck. “We recently provided folders to Family Medicine of Cloverdale, and we are offering them for free to the public and to all Putnam County health care providers.”
