PCHPCA: Advance care planning opportunities set
The Putnam County Hospice and Palliative Care Association (PCHPCA), along with other national, state and community organizations, is leading the effort to highlight the importance of advance health care decision-making on National Healthcare Decisions Day on April 16.
As a participating organization, the PCHCPA provides information and tools to help people talk about their wishes with loved ones and health care providers, and execute written advance directives in accordance with Indiana state law. These resources are available at www.pchpca.org.
Local opportunities to learn about advance care planning will be offered as detailed below:
• Tuesday, April 4 – Free workshop from noon-1 p.m. Topic will be choosing your health care representative and talking to them. In the Colonnade Room at Gobin Memorial United Methodist Church. Register by 5 p.m. on April 2 at https://dusk-to-dawn.org/.
• Wednesday, April 12 – Free monthly advance care planning learning opportunity from noon-1 p.m. via Zoom. Register by email to contact.us@pchpca.org, or call 765-301-7614 for additional information.
• Friday, April 14 – The PCHPCA welcomes to the public to receive free information about advance care planning and advance directive forms from 10 a.m.-3 p.m. In the Putnam County Hospital lobby. Masks are required.
The Putnam County Hospice and Palliative Care Association seeks to enhance the quality of life for the terminally ill, their loved ones and caregivers by educating the community about the benefits of hospice, palliative care and advance care planning.
The PCHPCA is a nonprofit public charity that operates with the help of donations, financial support from the Putnam County Community Foundation and fiscal sponsorship by Putnam County Hospital.
