PCH Physician Practices offering sports physicals

Putnam County Hospital Physician Practices is offering back-to-school sports physicals during special hours now through the summer for area student athletes.

Physicals are given by appointment at any of the following three physician practices: Family Medicine of Cloverdale, North Putnam Family Healthcare and Putnam Pediatrics and Internal Medicine.

The fee is $25 and is not billed to insurance. Participants need to bring a list of all current medication with them. The goal is to make sure the student is physically able to play the sport for which he or she is pursuing.

Students should bring a copy of the IHSAA physical form to their appointment. This form can be picked up at their school or can be downloaded at http://www.ihsaa.org/Schools/Forms.
