ROACHDALE — A Roachdale woman has been identified in a fatal collision that occurred south of the town on Monday.

First responders were dispatched shortly after 8 a.m. for a motor vehicle incident with injuries and entrapment on County Road 250 East. A box truck and a van were found wrecked side by side off the roadway.

The driver of the van, Amanda Charters, 36, was pronounced dead at the scene. A passenger, Chance Charters, 9, was seriously injured and airlifted by StatFlight to Riley Children’s Hospital in Indianapolis. The driver of the box truck, Glenn Calvin Jr., 27, Indianapolis, was unhurt.

Apart from the Putnam County Coroner’s Office, an Indiana State Police reconstructionist was called to analyze the scene.

According to a report by the state police, Charters was traveling northbound when Calvin, going south, ran off the west side of the roadway. Calvin then came back onto the road and went left of center, after which Charters appeared to have steered off the east side and was struck.

Putnam County EMS and PMH Ambulance, as well as the Putnam County Sheriff’s Office, the Roachdale Volunteer Fire Department and the Roachdale Police Department, responded to the incident.

A GoFundMe page has been set up for Charters' family. The page can be found at http://tinyurl.com/2s6v994d.
